- Two small flash fires occurred, causing minimal damage to an atmospheric oil tank. The first one was focused on the atmospheric oil tank pressure vacuum relief as welders on the top deck of the platform were cutting and wielding in preparation to install a crane. The wet oil tank was located on the second deck and had been isolated from the cutting an wielding operations by handing tarps. At the time of the incident strong winds were coming from the southeast, it was reported that wielding sparks or hot slag was blown behind the wet oil tank protective tarps and ignited hydrocarbons coming from the vacuum relief. Workers responded to a loud noise, sounding like a small explosion on the second deck and observed smoke and vapors emitting from the wet oil tank vacuum relief. The top of the wet tank was sprayed with water until it was determined it was free of ignitions sources. No injuries or fatalities were reported. The tarp was moved to be lay on top of the pressure vacuum relief and sprayed with water. A second fire occurred when hydrocarbon gasses build up under the protective tarp ignited, the cutting and wielding operations were suspended again until the fire was put out.
- While offloading water to a Tri-Union Facility the M/V April bumped the facility and sustained a hole just above the water line. The vessel began to tank and fill with water, later it sank, no injuries or fatalities were reported. A sheen originated from the vessel, there were no efforts to recover the petroleum products spilled.
