Freeport McMoRan Accidents

There have been numerous reports of accidents occurring at Freeport McMoRan operated oil rigs, a few examples are as follows:

  • One explosion and subsequent fire occurred as a result of both equipment failure and human error.  There was a failure to follow hot oil heater start up procedures and the valve positioner on the actuator of a fuel gas system shut down valve was incorrectly reporting a fully closed position. No injuries were reported
  • A worker was attempting to swap a pressure valve and needed to do so by cutting the bolts under low visibility. A grinder with a cutting wheel was used in a small space which led to a kick back of the grinder causing a 4 inch laceration to the hand of the worker.
  • A worker was assisting in securing a flat pin into the well conductor casting as it was being held by a derrick barrage crane. A unforeseen wave caused a shift in the tension of the crane and the pin slid out of place striking the worker’s ankle causing a fracture.
  • A crew was attempting to replace a leaking flange gasket and after a corroded lower nipple broke, a worker tried to press sight glass, back to the blowdown separator, causing the top nipple to break, driving his index finger into a code plate bracket and resulting in a severe laceration.
