  • In 1997, a crane fell overboard on a Murphy Platform. As the workers begin to lift the crane, the crane had metal fatigue on the pedestal.   • In 1996, a crane operator was using the fast line with a nylon strap to relocate valves to the opposite side of the blowout preventer stack. There was slack in the nylon strap which caused travel in the fast line ball. High wind struck the ball which caused a section to fall from the rig floor and injured a worker’s hand as well as created a compound fracture to his leg and a laceration to his shoulder.
