  • On the way to a rig, a water vehicle collided with the caisson resulting in two men aboard the vehicle receiving minor injuries. Numerous vehicular accidents occur annually at oil rigs involving either boats or helicopters and most often happen because workers lack proper training or adverse weather conditions make it difficult to see conditions.
  • In 2001, a platform operator noticed smoke coming from the condensate heater-treater stack. The workers immediately shut in the heater-treater and found a small leak in the fire tube. Condensate and water were flowing into the fire tube, which was creating smoke. Given the large number of pieces of equipment that are used on oil rigs, there are many accidents that occur there each year involving workers who are not properly trained to use equipment or machinery that is not properly maintained.
  • In 2005, operators set up to charge a battery from the back of the MCC and ran an air condition electrical supply cord and charging cables through a crack in the back MCC room door. The back door stop failed and the door closed on the air condition cord and charging cables, which cut the charging cables and resulted in an electrical short and ignited the cables. Many electrical fires on oil rigs occur because workers fail to properly adequately maintain machinery and equipment.
